To avoid delays in payment, it’s crucial to ensure that the confidentiality release form is signed ON or BEFORE the first date of services.

Important: We cannot bill for any services provided before this form is signed.

Example: If you conduct a prenatal visit on July 2nd, but the confidentiality release form is dated July 14th, we will be unable to bill for that July 2nd visit. The form must be signed before or on the same day as your first service to ensure that all services are billable.
